Sweet Adelines International

The membership of Midwest Gateway Region 5 includes many outstanding Sweet Adelines who have given selflessly of their time, talents and expertise for the love and joy of singing and have had a profound impact on the entire membership of Region 5. Of these, one is the person worthy of the Hall of Fame Award for Region 5. The annual Hall of Fame Award is a very prestigious, once-in-a-lifetime award.

Nominations

Individual members of Region 5 are encouraged to submit a nomination(s) for the Hall of Fame award. The winner will be honored and inducted into the newly established Region 5 Hall of Fame during the Regional Convention weekend. Each year's winner will be chosen from the nominations by the previous three Hall of Fame winners, but until that time, by members of the Regional Management Team. A nomination form, a written narrative and photo must be submitted with each nomination.

Qualifications

Please use the following qualifications as guidelines for nominees:

•  Displays enthusiasm and commitment to promoting the “Harmonize the World” theme of Sweet Adelines International and Region 5 through her music and interpersonal skills

•  Actively participates in both chorus and regional activities and functions

•  Willingly shares skills and knowledge with other members

•  Successfully completes volunteer assignments

•  Willingly works without expectations of recognition

•  Minimum of five years' membership in Sweet Adelines International

Nomination Process

•  Completed nomination forms must be received by TBA.

•  Use the attached form to submit nominations. Make extra copies as needed.

•  Include a 3x5 or 4x6 color photograph of the nominee with the form.

•  Include a written narrative that explains and encourages participation for the Hall of Fame. The narrative may be used in the presentation, should your nominee be a winner.

•  Send the nomination form, photo and narrative to TBA.
